GOLD STAR ATHLETICS POLICIES          AND RELEASE STATEMENT

PART 1
Payments: Payments are due by the scheduled dates provided on the original invoice.

Uniforms: Players should wear appropriate attire (shorts, sweats, workout pants, t-shirts, etc.). No cleats are allowed. No jewelry. Helmets are required at all times during live Hitting stations.

Withdrawals & Make-Ups: No credits or refunds will be given either for classes/rentals not attended or withdrawing from the program prior to the registrant’s expiration date. No Make-ups.

Valuables: Valuable items should not be brought to the facility. The LLC is not responsible for lost or stolen items.

Classes: Do not drop a participant off more than 15 minutes prior to the beginning of a session, League, clinic or other program.

Visitors: Visitors must sit quietly in the designated areas. Please do not talk to or otherwise disrupt the participants. Any interruption could result in serious injury.

Participant: Any person that engages in any LLC activity as a guest, athlete, volunteer and staff.

Participation: The LLC reserves the right to cancel a participant’s participation at any time. All participants now have to reserve cage time. Turf is only used for classes, teams or if reserved ahead of time.

Food & Drink: NO food or beverages are to be brought into any of the instructional (green) areas. Please make every attempt to keep food and drinks in the tan floor area.

The LLC’s Gold Star Athletics program is truly a state-of-the-art facility. We expect players and coaches to work hard to ensure that instructional areas are to be organized and picked up after use. Thank you for your efforts.

PART 2
I fully understand that the Gold Star Activities LLC (“LLC”) and Centre For Arts & Athletics (“Centre”) staff members are not physicians or medical practitioners of any kind. With the above in mind, I hereby release the above mentioned staff to render temporary first aid to me, my child or children in the event of any injury or illness, and if deemed necessary by the staff, to our doctor and seek medical help, including transportation by a staff member or its representatives, whether paid or volunteer, to any health care facility or hospital or the calling of an ambulance for said child should the staff deem this to be necessary.

PART 3
We, the staff of the “LLC” and “Centre” recognize our obligation to make our participants and parents aware of the risks and hazards associated with Athletic Sports. Participants may suffer injuries, possibly minor, serious or even death. Athletic Sports can be dangerous and can lead to injury! Parents should make their children aware of the possibility of injury and encourage their children to follow ALL the safety rules and coaches instructions. The “LLC” and “Centre”, its coaches & other staff members, will not accept responsibility for injuries sustained by any participant during the course of Athletic Sports instruction, or open workouts or in the course of any practice, competition or clinic in which he or she may participate or while traveling to or from the event. With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the programs offered by the “LLC” and “Centre”. I, my executors or other representatives, waive and release all rights and claims for damages that I or my child have against the “LLC” and “Centre” and/or its representatives whether paid or volunteer.

I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage that I consider adequate for both my and my child’s protection. I also understand that it is the parent’s responsibility to warn the child about the dangers of Athletic Sports and injury. The parent should warn the child according to what the parent feels is appropriate.

PART 4
The LLC may use my child’s photograph on their website and for promotional purposes.

                                                                                   RULES OF CONDUCT

  • These rules shall not be construed to be all-inclusive, but should be viewed as a guide for other situations, which may occur.
    These rules apply equally to everyone while on CENTRE FOR ARTS & ATHLETICS (“Centre”) and GOLD STAR
    ACTIVITIES LLC (“LLC”) property. Violation of the rules may result in suspension from use of facilities.
  • ILLEGAL FIREARMS OR ANY WEAPONS ARE PROHIBITED AT ALL “CENTRE” and “LLC” FACILITIES.
  • The use or possession of any illegal alcoholic beverages or illegal drugs is strictly prohibited.
  • Glass or metal containers (except those associated with normal operation of the facility) are prohibited due to safety, littering and
    equipment damage considerations. Everyone is responsible for ensuring that all debris is removed from the facilities.
  • Property abuse will not be tolerated. This shall include all “Centre” and “LLC” equipment (bats, helmets, mats, etc.) and the
    facilities.
  • Careless and reckless driving is strictly prohibited. There will be no parking near the entrance gate to the facility. Vehicles will
    be towed at the owners expense.
  • Horseplay is prohibited. This is to include sliding on handrails, fighting and sliding on hills.
  • Warm ups outside designated areas are prohibited due to potential for injury and property damage.
  • Batting or throwing against backstops or fences is prohibited due to damage considerations. Climbing over fences or on fences,
    batting cages, lockers, curtain walls, or backstops is prohibited due to safety considerations. Anyone observed abusing these
    assets of the facility will be suspended from use of the facility.
  • All participants shall maintain and return the facility to the way they found it. This includes REMOVING trash and garbage
    from the premises or disposing it in trash cans and recycle bins.
  • Verbal abuse or threatening language directed toward anyone, pushing, shoving, hitting, and similar activities will not be
    tolerated and may result in suspension or termination of use of the facilities.
  • Parents/Guardians shall supervise non-participating children at all times while on the premises.
  • All riding toys, including trail bikes, three wheelers, skateboards and roller blades are prohibited, except in authorized areas.
  • Pets are prohibited on the premises.
  • Any participant under a doctor’s care for injury or illness must present a medical release from their doctor to their respective
    Coach, trainer, or instructor before they can participate in games or practices.
  • “Centre” and “LLC” or affiliates are not responsible for personal injury or damage to any motor vehicle while on “Centre” and
    “LLC” property.
  • All injuries must be reported immediately.
  • Any coach, trainer, instructor, or parent ejected from a game or practice by a “Centre” or “LLC” Official shall leave the
    building and remain at their vehicle until their child finishes play. Any player ejected from play must leave the premises
    with their parents. If no parent is present, the player must go to the waiting area until a parent arrives to pick them up. An
    additional offense will result in suspension of use of the facilities for the remainder of the year.
  • All parents shall remain in designated parent waiting area. THIS MEANS NO STANDING IN WALKWAYS
